OK, I see what the problem is. This is a Digikam bug.

Digikam preserves the file's modification time when rotating, even though the
file has clearly been modified. Therefore, the thumbnailing code in KIO has no
chance to detect that the preview that is stored in ~/.thumbnails (which
appears to be newer than the image!) is outdated.
