Do you have change anything before that ?

I personally encounter this kind of behavior when set digikam to read  
metadata from XMP sidecar, and since exif info are not included in XMP,  
exif rotation was simply not applied...

Indeed, if I add tags, caption etc. to a raw file, that data is stored in a
sidecar, and the RAW Exif data aren't read anymore. But, if you use the menu
'Image -> Adjust Exif rotation tag' you can get the proper orientation back.
